Notice of Privacy Practices

We are required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. This notice is effective August 2, 2026 and applies to all of the health information we hold about you.

• For treatment. To provide, coordinate and manage your care, including sharing information with other providers involved in your treatment.
• For payment. To bill and collect payment for the care you receive, including working with financing partners you have chosen to use.
• For health care operations. To run the practice: quality review, training, and business management.
• Appointment reminders. To contact you about appointments, using the contact details and methods you have agreed to.
• As required by law. Where disclosure is required by federal, state or local law.
• Other uses permitted by law.We may also use or disclose your information without your authorization for public health activities; to report suspected abuse, neglect or domestic violence; for health oversight activities; in response to a court order, subpoena or lawful law-enforcement request; to coroners, medical examiners and funeral directors; for organ donation; for research approved by a review board; to avert a serious threat to health or safety; for specialised government functions including military and national security; and for workers’ compensation claims. Any use or disclosure not described in this notice will be made only with your written authorization.

We will obtain your written authorization before using or disclosing your information for marketing, before selling any of it, and before using your photographs for promotional purposes. You may revoke an authorization in writing at any time, and that revocation will apply going forward.

• To inspect and receive a copy of your health information.
• To ask us to correct information you believe is incorrect or incomplete.
• To request confidential communications, for example that we contact you only at a particular number.
• To request a restriction on certain uses and disclosures. We are not required to agree to most requests, but we must agree to a request not to disclose information to a health plan for a service you have paid for in full out of pocket.
• To receive a list of certain disclosures we have made.
• To receive a paper copy of this notice, even if you agreed to receive it electronically.
• To be notified if there is a breach of your unsecured health information.

If you believe your privacy rights have been violated, you may complain to us using the details below, or to the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
